Live Scan Application Form

The Live Scan application form is required for electronic fingerprinting, which is a key step in the background check process for your CCW permit. This form includes your personal information and agency details, and it is submitted by a certified Live Scan provider directly to the Department of Justice (DOJ) and/or FBI. Please do not complete fingerprinting until you are instructed to do so by a Sheriff’s designee.
